This accessory with retractable fingers that flare out during withdrawal to expose proximal sides of haustral folds may improve adenoma detection.

There is great interest in technologies designed to improve adenoma detection rates during colonoscopy. One recent example is the Endocuff, a disposable accessory that fits on the end of the colonoscope. It has retractable fingers that collapse during insertion but flare out during withdrawal to straighten the colon and expose mucosa on the proximal sides of haustral folds.

To assess the performance and safety of the device, which increases the diameter of the colonoscope, investigators in Germany retrospectively analyzed 50 Endocuff-assisted procedures. The cecal intubation rate was 98%, the mean cecal intubation time was 6 minutes, the ileal intubation rate was 76%, and the adenoma detection rate was 34%. Evidence of mucosal trauma was observed, but no serious complications occurred.
